While being transferred from a surgical table to a patient
lift after her surgery, Ms. Feldman was dropped on the floor. Simultaneously, the top of the surgical table
fell to the floor.
We investigated this loss, tested the table operation,
demonstrated the sequence of events with a crash-test dummy on the same
equipment, and demonstrated that a loose table top could not have caused the
fall. The fall was caused by tightening
one side of the patient lift sheet while the other side was loose; this caused
the patient to roll off the side of the table.
